Before you start
- Save your business profile and branding so the public form uses your identity.
- Decide which services this form should offer. A wedding page can offer only weddings; a general contact page can offer several categories.
What you will learn
- Show relevant questions after a visitor chooses a service.
- Use a shareable link or website embed without building a separate form service.
- Review the submission and reply without exposing internal notes.
Step-by-step
Create a form and its service choices
Open Contact Forms and add or edit a form. Choose the categories you offer and give the form a useful title and introduction. Use separate forms when different website pages should present different service choices.
Write questions for each category
Add section headings and text, single-choice, or checkbox questions. Reorder them into a natural conversation: what they need, relevant project details, then contact information. First name, last name, email, and phone collect the information needed to respond.
Preview and save
Check every offered category rather than only the first. The form follows the logo and colours in Business profile & branding; it does not need a separate palette. Save your changes to publish them—an unfinished editor draft is not the public form.
Place the link or embed on your website
Copy the form's share link or embed code into the appropriate page. A category-specific form belongs on a matching service page. Test it as a visitor before linking it prominently, using clearly labelled test details rather than real client information.
Review the inquiry and follow up
Read the submitted details in the inquiry inbox. When email delivery is configured, the business receives a formatted notification; Reply in your email app goes to the inquirer's address. Create or link the contact and turn the reviewed inquiry into a Lead project with the appropriate workflow.
Contact Form or Booking Request Form?
Use a Contact Form when a visitor needs to explain what they are looking for before you discuss price or scheduling. Use a Booking Request Form when they should choose a package and an available date/time. Neither form is an automatic confirmation of a job.
- A wedding inquiry can ask about venue, date, coverage, and preferences.
- A headshot inquiry can ask about team size and intended use.
- Keep private rates, staff notes, and internal workflow comments out of public question wording.
The reply is for the client; the record is for you
The notification's reply address lets you use your normal email app to respond. Review the message before sending: do not quote internal-only comments, workspace administration details, or unrelated client information. A notification is not a full two-way email inbox inside Flovuni.
- Email notifications depend on the business's sending setup.
- An inquiry submission does not charge a card or create a paid booking.
- Check the saved submission when an email is delayed or missing.

Common questions
Can I ask different questions for weddings and portraits?
Yes. Category-specific questions appear for the client's selection. Restrict a form's offered categories when only one service belongs on that website page.
Can I reply directly to the notification email?
When the inquiry email is configured and delivered, its Reply-To is the inquirer. Check the recipient before sending your reply.
